Use this form if you have multiple sources of income (W-2s, 1099s, retirement income, investment income). This worksheet helps you track all income sources so nothing gets missed on your tax return.

The Oregon Kicker is a unique tax credit that refunds excess revenue when state collections exceed forecasts by 2% or more. When this happens, Oregon law requires the state to return the surplus to taxpayers. This appears as a credit on your Oregon state return and can significantly reduce what you owe or increase your refund.
In simple terms: When Oregon collects more tax revenue than projected (exceeding their forecast by 2% or more), eligible taxpayers receive this as a credit on their state return.
Portland-area residents need to be aware of two voter-approved local taxes:
Metro Supportive Housing Services Tax: This tax funds homeless services and affordable housing. It's calculated as 1% of taxable income over $125,000 for individuals ($200,000 for joint filers).
Portland Arts Tax: This is a flat $35 annual tax for Portland residents 18 and older who have income above the federal poverty level. It promotes art in the Portland metro area.
Both taxes were voter-approved and remain in effect until changed by future votes.
